PERSONAL HISTORIES PROJECT
An Information Sheet for Work Group Members

General Description

Work Group members will contribute to the identification of ASAIO pioneers and help identify priorities for the personal histories project by serving as the initial contacts. Work Group members will personally contact select individuals and inquire into the existence of historical documentation - that is, where are their personal papers, laboratory records or devices? Are they being preserved (and if not, certainly Project Bionics can assist.) Members will also inquire into existing interviews conducted on these pioneers. These interviews will be reviewed, and will contribute to oral and video history planning for the purposes of our project.

Information Sought from Pioneers:

• Location, address, email address of pioneer
• Current position
• Full CV and Bibliography - request a copy; preferably an electronic copy that can be sent to the Project Bionics office for a database
• Birth Date and Family information
• Biographies, biographical sketches, interviews - paper, oral, video?
• If not in CV - information regarding grants, patents, contracts, membership, officer positions in ASAIO, ISAO, ESAO, JSAO
• Laboratory - active? Location, artifacts and devices? Contact person in lab?
• Clinical - major hospital affiliation? hospital historical displays? Organized patient groups? Special patients?
• Personal papers - location, plans for preserving/ archiving?
• Meetings where a recorded interview might take place?
• ASAIO - June 13-15, 2002 in New York?
• Other: __________________________

When collected, information can be forwarded to ASAIO Scholar at Project Bionics Office for database management.
